By downloading or using DisabilityLog (the "App"), you agree to these Terms of Service. If you do not agree, do not use the App. The App is published by an independent iOS developer (Apple Developer Team ID ZM8LF8494F); references to "we," "us," and "our" mean that developer.
DisabilityLog is a personal daily journal for people applying for or maintaining disability benefits (SSDI, SSI, Long-Term Disability, VA disability, and similar programs). You enter information about your daily function, symptoms, medical appointments, and medications; the App helps you organize that information and generate reports for your own use, including reports formatted for an attorney or for an Administrative Law Judge (ALJ) hearing.
The App is free to download and use with the following limits: daily check-in (function level 0-10 + day type), up to 10 daily log entries, 7-day function trend on the Home dashboard, read-only SSA-3373 category reference guide, and basic claim setup.
Pro features unlock when you start a Pro subscription. Two products are offered:
Both products unlock the same set of Pro features. Prices shown are USD; Apple displays the equivalent local price at purchase time.
Subscriptions automatically renew until canceled. Renewal is charged within 24 hours before the end of the current period unless auto-renewal is turned off at least 24 hours before that end. Payment is charged to your Apple ID. You can manage and cancel your subscription in Settings > [your name] > Subscriptions on your iOS device. Refunds for the unused portion of an active period are not offered by DisabilityLog; refund eligibility is determined solely by Apple under its published policies.
If you start a free trial and cancel during the trial, you keep Pro access through the end of the trial period and are not charged. If you do not cancel, your subscription begins at the end of the trial at the published price.
Pro provides the following features. The behavior described here is binding on us — if a future App update materially changes any of these descriptions, we will revise this section.
The following are foundational user controls and are available to all users at no charge: Face ID / Touch ID app lock, home screen widget, Good Day / Bad Day calendar view, free Markdown summary export, and the daily check-in reminder.
The App is provided "AS IS" and "AS AVAILABLE" without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, accuracy, or non-infringement. To the maximum extent permitted by law, our aggregate liability arising out of or relating to the App is limited to the amount you paid us, through Apple, for the App in the twelve months preceding the claim. We are not liable for indirect, incidental, special, consequential, or punitive damages, including denied benefits, lost income, lost data, or claim disputes. Reports generated by the App are tools for your use; we do not guarantee that the Social Security Administration, the Department of Veterans Affairs, any insurance carrier, any attorney, or any Administrative Law Judge will accept, weigh, or rely on them in any particular way.
Your use of the App is also governed by Apple's standard End User License Agreement (EULA) for licensed applications, available at https://www.apple.com/legal/internet-services/itunes/dev/stdeula/. In the event of a conflict between these Terms and the Apple EULA, these Terms govern as between you and us; the Apple EULA governs as between you and Apple.
We may update these Terms from time to time. Material changes will be surfaced in the App on next launch. Continued use of the App after a change constitutes acceptance.
These Terms are governed by the laws of the United States and the State of California, without regard to conflict-of-law principles. Disputes will be resolved in courts located in California, except that either party may seek injunctive relief in any court of competent jurisdiction to protect intellectual property rights.
