By downloading or using GlucoCompanion (the "App"), you agree to these Terms of Service. If you do not agree, do not use the App. The App is published by Flint Software, an independent iOS developer (Apple Developer Team ID ZM8LF8494F); references to "we," "us," and "our" mean that developer.
GlucoCompanion is a personal records organizer with insurance reference content and a deterministic appeal deadline calculator. You enter information about your diabetes profile, your prior authorization (PA) and appeal encounters, and your manufacturer Patient Assistance Program applications. The App helps you organize that information, look up reference content (Medicare LCD/NCD criteria, Medicaid 50-state coverage rules, manufacturer PAP eligibility), compute appeal deadlines, and generate PDF reports for your own use.
The App is free to download and use, with the following limits: 1 Diabetes Profile, 10 PA / Appeal Encounter entries, read-only Reference Mapper lookup, the basic 8-organization Patient Advocacy directory (ADA, JDRF, Beyond Type 1, Beyond Type 2, PAF, PAN, SHIP, NAIC), 6 of 22 encounter category types, and a basic Encounter Summary PDF report.
Pro features unlock when you start a Pro subscription or complete the Lifetime purchase. Two products are offered:
Both products unlock the identical Pro feature set. There is no monthly subscription — diabetes is a multi-year journey, and PA cycles do not fit monthly cadences. Prices shown are USD; Apple displays the equivalent local price at purchase time.
The Annual subscription automatically renews until canceled. Renewal is charged within 24 hours before the end of the current period unless auto-renewal is turned off at least 24 hours before that end. Payment is charged to your Apple ID. You can manage and cancel your subscription in Settings > [your name] > Subscriptions on your iOS device. Refunds for the unused portion of an active period are not offered by GlucoCompanion; refund eligibility is determined solely by Apple under its published policies. The Lifetime purchase is a single charge with no recurring billing.
If you start the 14-day free trial and cancel during the trial, you keep Pro access through the end of the trial and are not charged. If you do not cancel, your subscription begins at the end of the trial at the published price.
Pro provides the following features. The behavior described here is binding on us — if a future App update materially changes any of these descriptions, we will revise this section.
The App is provided "AS IS" and "AS AVAILABLE" without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, accuracy, or non-infringement. To the maximum extent permitted by law, our aggregate liability arising out of or relating to the App is limited to the amount you paid us, through Apple, for the App in the twelve months preceding the claim. We are not liable for indirect, incidental, special, consequential, or punitive damages, including lost data, lost insurance recoveries, denied PA outcomes, missed appeal deadlines, lost Patient Assistance Program eligibility, lost insurance coverage, or any clinical outcome. Reference content and deadline calculations are tools for your use; we do not guarantee that any insurance carrier, government agency, or court will accept or be bound by them.
Your use of the App is also governed by Apple's standard End User License Agreement (EULA) for licensed applications, available at https://www.apple.com/legal/internet-services/itunes/dev/stdeula/. In the event of a conflict between these Terms and the Apple EULA, these Terms govern as between you and us; the Apple EULA governs as between you and Apple.
All product, organization, and agency names referenced in the App and in this document — including but not limited to Medicare, Medicaid, ERISA, Eli Lilly, Novo Nordisk, Sanofi, Dexcom, Tandem, Insulet, Omnipod, Abbott, Roche, ADA, JDRF, Beyond Type 1, Beyond Type 2, Patient Advocate Foundation, Patient Access Network Foundation, SHIP, NAIC, Aetna, BCBS, UnitedHealth, Cigna, Humana, and Kaiser — belong to their respective holders. GlucoCompanion is not affiliated with, endorsed by, or connected to any listed entity.
We may update these Terms from time to time. Material changes will be surfaced in the App on next launch. Continued use of the App after a change constitutes acceptance.
These Terms are governed by the laws of the United States and the State of California, without regard to conflict-of-law principles. Disputes will be resolved in courts located in California, except that either party may seek injunctive relief in any court of competent jurisdiction to protect intellectual property rights.
