InfusionLog Terms of Service

1. Acceptance of these terms
By downloading or using InfusionLog (the "App"), you agree to these Terms of Service. If you do not agree, do not use the App. The App is published by an independent iOS developer (Apple Developer Team ID ZM8LF8494F); references to "we," "us," and "our" mean that developer.
2. What InfusionLog is — and is not
InfusionLog is a personal documentation tool for tracking specialty pharmacy shipment timelines, service-level agreement (SLA) breaches, customer service calls, missed-dose health impacts, and complaint filings. You enter information about your specialty pharmacies, shipments, calls, and health consequences; the App helps you organize that information and generate PDF reports for your own use, including reports you may share with an advocate, attorney, state insurance commissioner, or accreditation body.
InfusionLog is not legal, financial, medical, or insurance advice, an insurance product, a connected service, or a medical device. We do not sell or recommend specialty pharmacy services. We do not appraise medications, services, or health-care charges. We do not file complaints or appeals on your behalf. We do not communicate with any specialty pharmacy, insurance carrier, pharmaceutical company, accreditation body, shipping carrier, regulatory body, or court. We do not log into your specialty pharmacy on your behalf. We do not generate complaint letters or appeal letters. All values, dates, breach categorizations, call quotes, health impacts, and high-level interpretations are entered, edited, and interpreted by you. The 11-category SLA breach library and any in-app text about state insurance commissioner procedures, ASHP / URAC accreditation processes, or call-recording state laws is general information only and is not legal, financial, medical, or insurance advice. Consult a licensed advocate, attorney, or healthcare provider for your specific case.
3. Subscriptions and the Pro tier
3.1 Free tier
The App is free to download and use with the following limits: 1 specialty pharmacy, up to 10 shipment records, basic shipment timeline view, up to 3 SLA breach entries, plain-text Markdown export of one shipment record, and Face ID / Touch ID app lock. The App is local-only; records are stored only on your iPhone, and user-data survival is via Apple's standard iOS device backup.
3.2 Pro subscription
Pro features unlock when you start a Pro subscription. Two products are offered:
- Pro Annual — $29.99 per year, with a 7-day free trial for first-time subscribers.
- Pro Monthly — $4.99 per month, no trial.
Both products unlock the same set of Pro features. Prices shown are USD; Apple displays the equivalent local price at purchase time.
3.3 Auto-renewal and billing
Subscriptions automatically renew until canceled. Renewal is charged within 24 hours before the end of the current period unless auto-renewal is turned off at least 24 hours before that end. Payment is charged to your Apple ID. You can manage and cancel your subscription in Settings > [your name] > Subscriptions on your iOS device. Refunds for the unused portion of an active period are not offered by InfusionLog; refund eligibility is determined solely by Apple under its published policies.
3.4 Free trial forfeiture
If you start a free trial and cancel during the trial, you keep Pro access through the end of the trial period and are not charged. If you do not cancel, your subscription begins at the end of the trial at the published price.
3.5 No cloud storage
InfusionLog does not use Apple's CloudKit framework and does not sync records across your devices. All records are stored only on your iPhone. User-data survival is via Apple's standard iOS device backup (iCloud Backup or iTunes / Finder); records are included in your device-backup snapshots under Apple's control and are restored when you set up a new iPhone from the backup. The developer of InfusionLog has no access to your data, your device backups, or your iCloud account.
4. Pro features
Pro provides the following features. The behavior described here is binding on us — if a future App update materially changes any of these descriptions, we will revise this section.
- Unlimited pharmacies and shipments: Pro subscribers may track an unlimited number of specialty pharmacies and an unlimited number of shipment records across multiple years. Records are kept private to the device.
- State Insurance Commissioner Complaint Pack: Generates a PDF compiling the user's shipment timeline, SLA breaches, communications, missed-dose health impact, and attached photos in a format the user may submit to a state insurance commissioner. The app does not file the complaint on the user's behalf, does not transmit content to any agency, and does not represent the user before any regulator. Informational only - not legal advice.
- Customer Service Call Log: Stores user-written summaries of phone calls with the specialty pharmacy - duration, hold time, representative name, key quotes the user heard, call outcome. The user is responsible for the accuracy of every entry. The app does not record calls; if a recording is attached, the user is responsible for compliance with their state's call-recording laws (some states require two-party consent).
- Missed Dose Impact Log: Stores user-entered records of doses missed due to specialty pharmacy failures, including symptoms experienced, provider consultations, ER visits, and hospital admissions. Health and cost values are entered, edited, and interpreted by the user. The records are personal evidence; they are not medical records of a covered entity under HIPAA and do not constitute medical advice.
- SLA Breach Tracker: Stores user-entered breach records across eleven structured categories. The categories are descriptive labels chosen by the user; the app does not adjudicate whether a service-level commitment was actually breached and is not an authoritative determination of any pharmacy's contractual obligations.
- Additional PDF reports: Four PDF formats generated entirely on-device using PDFKit. (1) ASHP / URAC Complaint Pack — formatted for submission to a specialty pharmacy accreditation body; the app does not submit complaints, is not affiliated with ASHP or URAC, and makes no representation about accreditation-body acceptance, response, or outcome. (2) Shipment History Report — multi-year compiled summary across one or more pharmacies. (3) Missed Dose Impact Report — causal-chain entries with health consequences and user-entered cost estimates; cost values are not appraisals. (4) Customer Service Audit Report — cumulative hold-time / call-duration totals and outcome breakdown by representative. No content is transmitted off-device during generation. Informational only — not legal, medical, or insurance advice.
- Document Vault: Imports user-supplied PDFs and uses Apple's VisionKit framework on-device to scan paper documents to PDF. Scanned and imported files are stored only in the app's local sandbox. No document content is transmitted to any developer-controlled server.
- Matrix Schema Coordination: Exports a file the user may import into a sister app (FlareLog, StepLog, or AuthGuard). Files flow through the iOS Files / Share Sheet under the user's control; no content is transmitted to any developer-controlled server. The sister apps are independent products and must be installed separately by the user.
- Archive backup: Pro subscribers may export the entire local archive to a single JSON file via the iOS Share Sheet. The file is generated on-device using JSONEncoder; no content is transmitted to any developer-controlled server. The file is the user's own backup; InfusionLog does not commit to backward-compatible re-import across major version bumps.
- Widget: Reads from local data only via App Group container. The widget renders neutral text (counts and integer minutes) and does not display pharmacy names, medication names, or estimated costs.
Free features. The following are included in the free tier and are never sold as upgrades: Face ID / Touch ID app lock; basic shipment timeline view; 1 specialty pharmacy + up to 10 shipment records; up to 3 SLA breach entries; and plain-text Markdown export of one shipment record. Privacy controls and basic functionality are foundational, not paid features.
5. Your responsibilities
- You are responsible for the accuracy of every value, photo, date, breach category, call quote, health impact, and cost estimate you enter.
- You are responsible for compliance with your state's call-recording laws if you attach an audio recording to a Customer Service Call Log entry. Some U.S. states require two-party consent for recording.
- You are responsible for maintaining standard iOS device backups (iCloud Backup or iTunes / Finder) so that records survive device loss. InfusionLog is local-only and has no remote copy of your data. Pro subscribers may additionally export the full archive as a JSON file via the Share Sheet for portable backup outside Apple's device-backup system.
- You will not use the App for any purpose other than personal documentation of your own specialty pharmacy records, or those of family members for whom you are the recognized caregiver, payer, or guardian.
6. Disclaimers and limitation of liability
The App is provided "AS IS" and "AS AVAILABLE" without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, accuracy, or non-infringement. To the maximum extent permitted by law, our aggregate liability arising out of or relating to the App is limited to the amount you paid us, through Apple, for the App in the twelve months preceding the claim. We are not liable for indirect, incidental, special, consequential, or punitive damages, including lost data, missed shipment deliveries, missed doses, lost insurance recoveries, dispute outcomes, denied treatments, or amounts owed to providers, carriers, or pharmacies. Reports generated by the App are tools for your use; we do not guarantee that any patient advocate, attorney, specialty pharmacy, insurance carrier, accreditation body, regulatory body, or court will accept them.
7. No affiliation with specialty pharmacies, pharmaceutical companies, or carriers
InfusionLog is not affiliated with, endorsed by, or sponsored by any specialty pharmacy (including but not limited to Accredo, CVS Specialty, Optum Specialty, Walgreens Specialty Pharmacy, Briova, AllianceRx Walgreens, Amber Pharmacy, Diplomat, Foundation Care, or PerformRx). It is not affiliated with any pharmaceutical company. It is not affiliated with FedEx, UPS, DHL, or any shipping carrier. It is not affiliated with ASHP, URAC, or any accreditation body. Trademarks referenced in the in-app Specialty Pharmacy Catalog (a picker convenience populated from publicly known specialty pharmacy names) are the property of their respective owners.
8. Apple End User License Agreement
Your use of the App is also governed by Apple's standard End User License Agreement (EULA) for licensed applications, available at https://www.apple.com/legal/internet-services/itunes/dev/stdeula/. In the event of a conflict between these Terms and the Apple EULA, these Terms govern as between you and us; the Apple EULA governs as between you and Apple.
9. Changes to these terms
We may update these Terms from time to time. Material changes will be surfaced in the App on next launch. Continued use of the App after a change constitutes acceptance.
10. Governing law
These Terms are governed by the laws of the United States and the State of California, without regard to conflict-of-law principles. Disputes will be resolved in courts located in California, except that either party may seek injunctive relief in any court of competent jurisdiction to protect intellectual property rights.
